Independent Living, Inc., a progressive and dynamic cross-disability services organization and a leader in the provision of peer services in the Hudson Valley region is seeking to hire an Outreach Coordinator (Bi-lingual) working within our Options/CDPAS program.


The Outreach Coordinator (Bi-lingual) is an agent for Independent Living, Inc. and Independent Home Care, Inc. acting as liaison between the agency’s system of care, program recipients, Personal Assistants (PA), County Adult Services Units and Managed Long-Term Care (MLTC) agencies. Responsibilities include but are not limited to initiating contact with eligible consumers and new referrals, onboards new consumers and personal assistants, provide translation services, as well as being a backup to the VDC (Veteran Directed Care) program. Associate degree (or equivalent experience), two years of experience in disability related services, bi-lingual English/Spanish, clean driving record and proficiency with Microsoft Office.

DUTIES AND RESPONSIBILITIES:

  • The Outreach Coordinator (Bi-lingual) is an agent for Independent Living, Inc. and Independent Home Care, Inc. acting as liaison between the agency’s system of care, program recipients, Personal Assistants (PA), County Adult Services Units and Managed Long-Term Care (MLTC) agencies.  
  • Utilizes a working knowledge of 
  • Consumer Direction procedures.
  • Medicaid regulations
  • HIPAA regulations   
  • Develops and maintains a professional working environment in accordance with the Independent Living philosophy.
  • Establishes contact with eligible consumers; forwards introductory marketing packet; schedules home visits.
  • Initiates contact with new referrals and runs initial eligibility check to determine if Consumer is eligible for CDPA services.
  • On-boards new Consumers and new Personal Assistants both virtually and in person depending on the needs of the individual. Travel is required in an agency own vehicle for approved drivers.
  • Meets with consumers/families/PAs to provide program orientation, review program manual, intake information. Processes all paperwork with accuracy. Checks for understanding.
  • Schedules regular consumer telephone and/or in person visits to do the following: Monitors status of program; Answers questions regarding recruiting, interviewing and on-boarding Personal Assistants.
  • Forwards Consumer and Personal Assistant information documents to Options staff and Finance for entry in payroll and program database.
  • Assists with Office needs as required when not involved in immediate Outreach.
  • Replenishes program literature when needed.
  • Provides translation services both orally and written as needed.
  • Attends agency-sponsored in-service/trainings.
  • Backs up intakes for the Veteran Directed Care Program as needed.

SHIFT: 

  • Monday through Friday 9:00am-5:00pm.


Pay Rate:

$22.00/hr - $23.00/hr

Requirements

  • Excellent communication skills; both written and oral.
  • Proficient with software navigation.

  • Excellent organizational skills and ability to manage multiple priorities.
  • Strong teamwork and interpersonal skills.
  • Must be flexible with time-sensitive reporting requests.
  • Must be able to work independently on assigned tasks and assist with team projects when needed.
  • Willing to share ideas and work closely with manager and team members to achieve program goals.
  • Demonstrated ability to recognize the need for and facilitate connections between consumers and IL and other disability related services. 
  • Knowledge of local, statewide and national disability related issues and community dynamics.
  • Ability to schedule appointments; flexible and resourceful.
  • Working knowledge of mobile devices.
  • Associate degree (or equivalent experience).
  • Two years of experience
