MISSION STATEMENT: The Rehabilitation Services Department provides the highest quality standard of Physical
Therapy, Occupational Therapy, Speech Therapy, Pulmonary Rehab, Cardiac Rehab, Diabetes Education, Athletic
Training Services, and etc. to our community. Optimum care will be given without regard to age, race, color,
religion, gender, sexual preference, or national origin in a cost-effective environment.
WORKING ENVIRONMENT: Usual working space is a well-ventilated area. Requires standing, walking, turning,
bending, stooping, and lifting patients. Fine and gross motor skills. Environment may be stressful at times,
therefore ability to work under stressful conditions is necessary. Also always subject to bloodborne pathogens
in patient care. May have occasional exposure to radiation during radiology procedures and to dangerous
chemicals (cleaning and disinfecting agents).
ESSENTIAL JOB FUNCTIONS:
1. The supervisor will be responsible for covering all manager duties when the Director is not available/on
premises. a. The supervisor will also work with the Director to complete all duties in a timely manner. b. Must be capable of identifying situations that may adversely affect patient outcomes, compliance, and reimbursement. c. Have working knowledge of payors and their impact on Out-Patient Rehabilitation Clinics. d. The Supervisor will follow department policies and Director recommendations to correct problems in these areas. e. If guidance is needed, will seek guidance from the Director of Rehabilitation Services. f. Will perform daily tasks/duties as requested from the Director.
2. The Physical Therapist will provide evaluations to patients demonstrating accurate and timely documentation of diagnosis, history, objective measurements, special testing, establishing a need for skilled physical therapy and a plan of care. a. They will perform treatments assuring accurate documentation of all components of a SOAP note including billing with time and units with supporting documentation. b. They will perform re-certifications every 30 days for out-patients including updating of goals and modifications in frequency/duration. c. They will perform 10th visit/progress notes as needed meeting Medicare guidelines. d. They will complete discharges for patients within 30 days of expiring POC. e. They will utilize information targeting insurance allotment of visits, track visits and submits for insurance authorization in a timely manner. f. They will serve as supervising therapists to support staff including: Physical Therapy Assistants and Rehab Techs. g. Physical Therapists will also be competent in the areas/modalities of treatment that they provide.
3. Is responsible for providing supervisory functions for site of Rehabilitation Operations in doing the following tasks: a. Maintains the work schedules, assigns duties assuring work and schedules are distributed and assures optimal staffing to provide patient care. b. Assist with addressing staff concerns. c. Assures daily assignments/tasks are being accomplished and staff remain productive and efficient. d. Assist with development and adherence to all disciplines specific evidence based and standards of practice within the department. e. Ensure therapy service provision and documentation of services are compliant according to departmental guidelines/regulations. f. Assist with competency checklist completion. g. Provide input on annual performance appraisals. h. Works in collaboration with the director to acquire additional staffing as needed and assuring new hires are appropriately trained/serve as preceptor i. Collaborates with other departments to meet staff, clinic and organizational needs. j. Orders and receives supplies, works with outside vendors and contract groups to assure clinic needs are met. k. Performs quality assurance audits and patient satisfaction surveys routinely. Guiding staff in needed improvements. l. Maintains time and attendance logs, keeping Director aware of excessive occurrences that need attention. m. Collaborates with Director in poor employee performance matters. n.
