Major Responsibilities:

1. Identifying patient eligible for grant criteria via access through the inpatient psychiatric and medical units

2. Data collection and Entry

3. Building community relationships with outside referral sources

4. Providing reliable transportation for patients as needed

5. Providing case management and referrals to meet patient needs

6. Connecting patients to the UMass Harrington Healthcare Behavioral Health continuum of care

7. Providing follow up contact with patients while part of the Bridge Clinic

4. Maintains regular, reliable, and predictable attendance.

5. Performs other similar and related duties as required or directed.

Position Qualifications:

1. Experience with substance use disorders and mental health disorders preferred or willingness to learn.

2. Excellent communication skills and interpersonal skills.

3. Attendance at various staff meetings in accordance with grant requirements and to ensure program staff involvement.

4. Valid driver’s license and proof of insurance.

5. Strong organizational skills.

6. Willingness to work with a multi-disciplinary team.

License/Certification/Education:

Required: Master’s Degree in Human Services

Preferred: Social Work (LICSW) or Mental Health Counseling (LMHC, LMFT)

Experience/Skills:

Required: One year of experience with substance use disorders and mental health disorders preferred or willingness to learn.

Preferred: Experience with co-occurring disorders, Bilingual
