Job Responsibilities:
· Recruits, hires and orients qualified personnel, and directly manages Admissions Coordinators.
· Directs all daily patient referral and intake operations including providing direct oversight of the establishment and implementation of Admissions Department processes.
· Directs the implementation of improved work methods and procedures to ensure eligible patients who desire our care are admitted as soon as possible.
· Establishes and maintains positive working relationships with current and potential referral sources to bring a greater understanding of the hospice philosophy and support to patients and families that desire our care.
· Ensures seamless transition of patients to home care by providing direct oversight of patient education and preparation for home care, plan of care initiation, and coordination of care with multiple service providers.
· Builds and monitors community and customer perceptions as a high quality provider of services.
· Leads excellent customer service by maintaining comprehensive working knowledge of community resources and assists referral sources in accessing community resources for customers that are ineligible for hospice services at this time.
· Gathers, collates, and reports referral statistics including key customer referral trends.
· Provides leadership in strategic planning including identifying opportunities for additional or improved services to meet unmet customer needs.
· Maintains comprehensive working knowledge of hospice’s contractual relationships and ensures that patients are admitted according to contract provisions.
· Works in conjunction with the sales team in responding to customer request and concerns in a timely manner to ensure customer satisfaction.
· Participates in quality assessment performance improvement teams and activities
· Ensures accurate data entry of patient information into the electronic health record system.
· Assists in communicating regularly with each agency’s leadership and sales team regarding the number of pending patients and admissions.
· Ensures compliance with all state, and federal referral/intake regulatory requirements.
· Attends Growth meetings at each agency periodically.
· Assists the Administrator in the preparation of an annual budget for the Admissions department and monitors allocation of resources according to budgetary limitations.
· Knowledge of and adherence to all policy and procedures.
· Maintains compliance by documenting accurately and timely within the organization’s electronic health record.

· Knowledge of and adherence to the employee Code of Conduct and Code of Ethics.
Education and Experience:
· A minimum of 4 year college degree, registered nurse required.
· At least three years’ experience in hospice required
· Able to work late evenings and weekends occasionally
· Familiar with medical terminology
· Able to manage a hospice program component
