Why this role matters

The primary purpose of this role is to provide case management support to people who are experiencing homelessness, are at risk of homelessness or who live in low-cost or insecure accommodation and have multiple unmet health needs. Further, the role seeks to provide clients with support, system navigation, housing advice, advocacy, and links to the community. This role requires that service delivery targets and organisational standards are achieved, and funding body requirements are met

The Community Connections Program (CCP) is funded by Home and Community Care through the Commonwealth Department of Health to provide support to eligible persons living within the local government areas of, Boroondara, Knox, Manningham, Maroondah, Monash, Whitehorse, and Yarra Ranges Shire.

About the role

We are seeking a values-driven and proactive Case Manager to deliver flexible and assertive outreach case management to young people experiencing homelessness or at risk of experiencing homelessness.

This is a max-term (10 months) part-time (22.8 hours per week) position based in Sunshine, Victoria. Salary and conditions are in accordance with the SCHADS Level 5.

You will successfully

  • Provide a quality service to clients via the following processes and in accordance with funding guidelines, accreditation standards, and program specific frameworks: initial assessment, case planning, case plan reviews, case closure.

  • Provide holistic and sensitive support to clients of the service, that considers and responds to their individual needs and enables achievement of case plan goals.

  • Support clients to access, navigate and interact with various services and supports, depending on their individual needs and goals. • Develop effective and supportive relationships with clients that encourages self-determination and participation.

You will have

  • Relevant tertiary qualifications in social work, human services, aged care, disability, community services, or related field (diploma level minimum), and/or relevant experience

  • Relevant experience in a social service environment.

  • A current Victorian Employee Working with Children Check.

  • Current Victorian Driver’s Licence.
