Family Nurse Practitioner (Indiana)

We are seeking a passionate Family Nurse Practitioner (FNP) to join our team. In partnership with a major health plan, we are innovating to close gaps in care by delivering healthcare to underserved populations through real-time telehealth visits. You will provide telehealth visits for postpartum and well-child check-ups for children aged 3 to 21 in Indiana. Providers will not be responsible for acute care, chronic illness management, or medication prescribing. Instead, you will serve as the initial point of contact, referring patients for ongoing care as needed.
Schedule:Our program operates seven days a week (weekdays from 8 AM to 10 PM EST and weekends from 8 AM to 8 PM EST). To best accommodate our patients, we require providers to work at least four-hour shifts. Upon hire, we will collaborate with you to create a schedule that aligns with both your availability and program needs.

How you will make an impact

• Postpartum visits: Perform a postpartum assessment including the Edinburgh depression screening and refer for any needs discovered.
• Well-child visits: Assess achievement of age-appropriate developmental milestones and provide anticipatory guidance. Assess immunization status and refer to close gaps.
• Outbound calls: Reach out to patients who no-show appointments. Perform outbound calls to patients in any down time (ie no-shows or schedules not being filled).
• Collaborate with assigned physician.

What we are looking for

• Active Indiana license as a family medicine nurse practitioner or physician assistant.
• 12 months of related work experience preferred.
• Ability to enroll as a provider in state Medicaid program.
• Fast & reliable Wi-Fi (Starlink not compatible).
• HIPAA compliant workspace required – Dedicated workspace behind closed door.
• Other duties as assigned.
