Weekdays from 8-4:30 on a per diem basis; No weekends, nights or holidays.

Elliot Pharmacy provides high-quality and personal prescription services to patients in New Hampshire and Massachusetts, including those whose providers are outside the Elliot network. We offer a variety of value-added services, including vaccinations, adherence packaging, meds-to-beds, shipping, and non-hazardous compounding. In addition, we are a designated specialty pharmacy with access to many hard-to-find medications. Our pharmacists, technicians and front-store team are committed to building long-term collaborative relationships with our patients and helping them become and stay well.

About the Job

Collaborate with Primary care patients and staff, focusing on medication therapy management, disease state management and education as part of the patients overall healthcare plan.

What You’ll Do

  • Provide medication related pharmacy consult appointments for patients and caregivers in collaboration with primary care providers.
  • Create and participate in Collaborative Practice Agreements per State of NH requirements.
  • Document regarding patient activities and communicate with care team via electronic medical record.
  • Provides in-service education, drug information (DUE), and other educational services upon request to staff and patients. Maintains professional relationships with nurses, physicians and allied health care providers. Represent clinical pharmacy in committees as assigned.
  • Precept pharmacy students and residents in accordance with accreditation standards.
  • Maintain and continuously improve skills by continuously displaying willingness to learn and participate in all types of continuing education opportunities.
  • Review and monitor patient medication profiles to provide clinical interventions when appropriate. Provide recommendations and other requested drug information to members of the clinical team.

What You’ll Need

Professional Pharmacy Bachelor’s Degree Required. Doctor of Pharmacy Degree preferred.

Licensed Pharmacist by the state of New Hampshire. Board Certified in Ambulatory Care Pharmacy preferred.
