Responsibilities:

• Conduct comprehensive physical therapy assessments and develop personalized treatment plans to achieve patients' goals and improve their quality of life

• Provide one-on-one physical therapy interventions, including exercises, modalities, and education to patients and their caregivers

• Collaborate with our interdisciplinary team, including physicians, nurses, and social workers, to ensure comprehensive care plans

• Maintain accurate and timely documentation of patient progress, treatment plans, and billing information

• Develop and implement strategies to prevent hospital readmissions and improve patient outcomes

• Stay up-to-date with the latest evidence-based practices and guidelines in physical therapy

Requirements:

• Home Health Physical Therapist with a Doctor of Physical Therapy degree from an accredited program

• Current licensure as a Physical Therapist in the State of Virginia

• Strong communication and interpersonal skills

• Ability to work independently and as part of a team

• Home Health Physical Therapist with a proven track record of providing effective and compassionate care to patients in a home health setting
