As an RN, you will assist fellow caregivers in delivering exceptional care, from initial introduction to final send-off. In this role, you will interact directly with patients, families, and visitors, offering the opportunity to enhance their experience and supply healthcare information as needed. At Cleveland Clinic’s Euclid Hospital, our Chronic Care Clinics (CCC) are dedicated to supporting patients living with chronic conditions such as Chronic Obstructive Pulmonary Disease (COPD), Congestive Heart Failure (CHF), and Hypertension. We understand the challenges these diagnoses can bring, and our mission is to enhance patients' quality of life—whether they are newly diagnosed, managing a long-term condition or transitioning home after a hospital or skilled nursing facility stay. Our multidisciplinary team works collaboratively to bridge the gap between physician and hospital care through proactive health monitoring, education and personalized support. We strive to create a positive patient experience by fostering strong, ongoing relationships between patients and care teams, ensuring coordinated communication and continuity across all providers.

A caregiver in this role works part-time 7:30am to 4:00pm. Days/schedules will vary based on facility needs.

A caregiver who excels in this role will:  

  • Provide professional nursing care in an ambulatory setting.  
  • Assess health status, complete nursing assessments, perform telephone triage, and establish care plans.  
  • Administer medication and treatments.  
  • Initiate CPR and other emergency measures.  
  • Notify the physician and/or Licensed Independent Provider (LIP) of abnormal findings. 
  • Assist with specialized diagnostic procedures, therapeutic procedures, and specialized patient care equipment. 
  • Administer medications and treatments as ordered by physician/LIP and monitor patients for response.
  • Develop, evaluate, adapt and document health education and provide it to patients and families.  
  • Cover other nursing roles within the department.
  • Participate in quality initiatives, process improvements, and research activities to improve patient outcomes, patient experience and patient safety.   
  • Travel within the region to support physician clinics and surgical procedures, as needed. 

Minimum qualifications for the ideal future caregiver include:  

  • Graduate from an accredited School of Nursing  
  • Current state licensure as a Registered Nurse (RN)  
  • Basic Life Support (BLS) Certification through the American Heart Association (AHA) 
  • Advanced Cardiac Life Support (ACLS) through the American Heart Association (AHA)
  • 1 year recent experience as a RN
  • One year of recent experience as an RN OR LPN, Medical Assistant and/or other unlicensed clinical healthcare experience if applicant is a current Cleveland Clinic caregiver in an ambulatory setting transferring into an RN role but remaining in the same current ambulatory Cleveland Clinic setting OR a current Cleveland Clinic caregiver in a clinical role within the same specialty area transferring into an ambulatory area
