Responsibilities
We are looking for Physical Therapists to join our Home Health team that serves the special purpose of caring for Valley patients in the safest place to them, their home. In your role, you will work one-on-one with patients to establish and implement a treatment plan for their recovery. Additionally, you will collaborate with a diverse team, providing mentorship and collaborating with seasoned clinicians, recent graduates, and everyone in between, averaging 25-30 visits per week. To ensure effective patient care, you will utilize appropriate procedures, equipment, and techniques within the scope of physical therapy while also communicating effectively with other patient care team members to coordinate the care provided.

Qualifications
- Bachelor's Degree in Physical Therapy required
- PT - State of California Physical Therapist License or eligible for licensure required
- DL - Valid California Driver's License and driving record acceptable to Community Health System required
- BLS - Current Basic Life Support (BLS) for Healthcare Providers by American Heart Association (AHA) required
