PRIMARY PURPOSE OF POSITION:

The Director of Social Services is responsible for planning, organizing, and implementing social services programs to meet the psychosocial, emotional, and developmental needs of residents. This role involves working closely with residents, their families, and the interdisciplinary team to ensure a high quality of life, promote overall well-being as well as meeting State and Federal laws and regulations. The Director of Social Services will lead Chai Point in matters of resident advocacy, protection, and promotion of resident rights.

 

QUALIFICATIONS:

Experience: Minimum of three years of social work experience working in long-term care including at least one year of experience working directly with the elderly population.

Education: Master’s degree in social work from an accredited school.

Licenses & Certifications: Licensed by the State of Wisconsin or eligible for Licensure.

 

JOB DUTIES:

1. Resident Assessment and Care Planning. Conduct comprehensive psychosocial assessments for new residents. Develop and implement individualized care plans in collaboration with the interdisciplinary team. Regularly review and update care plans to reflect changes in residents' needs and preferences. Attend and/or lead care conferences.

2. Support and Counseling: Provide counseling, coordinate mental health services and support Residents and their families regarding social, emotional, and financial concerns. Facilitate support groups, outside appointments and educational programs for Residents and families.

3. Resource Coordination: Identify and connect residents with community resources, including healthcare services, financial assistance, and social programs. Assist residents and families in navigating the healthcare system and accessing necessary services.

4. Advocacy and Rights: Advocate for residents' rights and ensure compliance with State and Federal laws and regulations. Address and resolve resident concerns, grievances and complaints promptly and effectively.

5. Program Development and Implementation: Develop and oversee social services programs and activities that enhance residents' quality of life. Organize events, outings, and recreational activities that promote socialization and engagement.

6. Team Collaboration and Leadership: Work collaboratively with the interdisciplinary team, including nursing, activities, and administration, to provide holistic care.

7. Documentation and Reporting: Maintain accurate and up-to-date records of resident assessments, care plans, and progress notes. Prepare reports and documentation as required by facility policies and regulatory agencies. Audit resident records to ensure compliance.

8. Professional Development: Stay current with industry best practices, trends, and regulations related to social services in assisted living. Participate in continuing education and professional development opportunities.

9. Perform other duties as assigned.
