OVERVIEW: Founded in 1979 in direct response to rising rates of hunger and homelessness in North San Diego County, Interfaith Community Services works to break the cycle of poverty and homelessness by Helping People Help Themselves. Interfaith is the most comprehensive social services agency in North County, providing both immediate safety net services and long-term, life-changing programs designed to meet the holistic needs of people in crisis. Together, in partnership with 250+ diverse faith communities, more than 650 recurrent volunteers, and a growing staff of over 200, Interfaith provides a safe haven and pathway towards self-sufficiency for more than 17,000 community members annually.  


WHAT YOU WILL DO: This position will provide case management services to individuals experiencing homelessness in Oceanside, focusing on housing stability while in a Bridge Housing Program.   The Housing Stability Case Manager will provide additional direct services to include: landlord engagement, orientations, facilitate groups, case conferencing, conflict resolution and mediation, unit inspections, budgeting, co-creating a client-driven housing plan, housing identification services and support, and client advocacy (resources & referrals).

  • Completes pre-screenings with referring party to confirm eligibility criteria is met for participant.
  • Collaborates with referring party to complete program intake and support the creation of a team approach with referring party, Client and other supportive resources for housing goal progress.
  • Provides guidance and support to help program participants identify their own strengths, set achievable goals and follow through consistently to improve and achieve self-sufficiency and housing
  • Complete check-ins and engages in case management meetings with the participant and with their team.   Identifies and provides ensure basic needs/housing supplies are available and to assist in addressing any behavior issues that do come up.
  • Provides direct services to program participants including, advocacy, and housing stability case management, group facilitation, conflict resolution, mediation, and budgeting
  • Collects all necessary documentation needed to support housing stability plan
  • Provides information and referral assistance regarding available support from appropriate social service agencies and/or community programs
  • Assists program participants in co-developing  a strength-based housing stability plan that promotes permanent housing, self-sufficiency and sustainability.  
  • Assists in development of and encourage adherence to a personal budget through pro-active housing and budget counseling sessions; provides budget counseling and education to assist clients in establishing payment plans for bills and past debts and to assist clients in obtaining housing
  • Identifies participant strengths and barriers to stability, and assists participants in reducing barriers and linking to resources and services
  • Works and collaborates with internal and external parties (volunteers, interns, other program providers) to provide clients with housing resources
  • Assists homeless program participants in locating and securing housing of their choice
  • Participates in all staff meetings and trainings
  • Performs additional duties as assigned by supervisor

Requirements

EXPERIENCE / EDUCATION

  • Bachelor’s degree in Social Work, Social Services or related field preferred (or equivalent work experience)
  • 1+ years of relevant wor
