Want to join our Team? We are currently looking for an inpatient and home hospice physician who is an experienced collaborative leader who works in concert with Hospice of the Chesapeake’s (HOC’s) medical staff.  This physician models excellence during the entire course of care, beginning at admission through case management and eventually transitioning caregivers into bereavement services. 


Office Location: Waldorf, MD

Coverage Area: Charles and Calvert Counties

Status: 40 hours per week

Schedule:  5 8-hour days and rotational overnight and weekend call 

Salary: Competitive, commensurate with experience, education and credentialing         

 

This physician exercises care plan oversight and ensures the highest quality hospice care is delivered at HOC’s Charles Inpatient Care Center (CICC) facility in Waldorf, Maryland, where the position is based.  This physician leads the corresponding regional interdisciplinary hospice team and may support palliative and population health initiatives as needed.  They  provide daily direct patient care in the CICC facility and bedside care where patients reside including all levels of billable and administrative visits. They align best inpatient practices with the agency’s second inpatient care center in Pasadena, Maryland. 

 

This physician nurtures relationships with regional referral sources and provides education to internal and external learners including medical residents and APP students.  They participate in HOC-related grant/research activities. They collaborate directly with referring physicians around symptom management and Medicare hospice benefit eligibility. 

 

The inpatient and home hospice physician models excellence in all aspects of documentation and billing practices including the certification of terminal illness, visit notes, orders, and death certificates. They complete daily documentation within 24 hours of a patient encounter. This physician participates in a pool to provide on call for after hours, weekends, and holidays.

 

Main Responsibilities:

  • Provides all aspects of direct patient care to inpatients and home-based hospice patients as assigned.
  • Educates the medical community about the value of HOC services in an effort to grow the organization and promote utilization of HOC services.
  • Assists in running interdisciplinary (IDT) meetings. Collaborate with team managers to ensure IDT meetings are productive, comprehensively address patient/family needs, and all team members are encouraged to participate.
  • Provides internal and external education.
  • Makes home/residence and inpatient care center visits daily.  Provides patient/family medical support. Provides F2F visits for team patients if/as assigned.
  • Provides assistance to Medical Services leadership to ensure HOC’s practice is compliant with organizational policy, local, state and federal laws, regulations, standards, and requirements of regulatory and accreditation agencies (e.g. JC/DOH/CMS), including those governing professional practice, and contractual arrangements (e.g. research, grants).
  • Effectively participates in Quality/Performance Improvement (QI/PI) programs in collaboration with supervisor and/or project manager. 
  • Confirms patient eligibility for hospice services in accordance with admission/recertification criteria and per hospice policy. This includes CTIs (certificates of terminal illness), patient orders, and death certificates as assigned.
  • Supports the Chief Medical Officer and the Director of Medical Services in preparations for surveys, administrative review and/or peer reviews and in maintaining standards and procedures which comply with licensure and accreditation requirements.  Assists in implementing corrective action required as a result of survey/review findings.
  • Other duties as assigned.
