Position Overview

The Division of Mental Health is seeking to hire a Social Worker II for the Elizabeth Packard Mental Health Center located in Springfield, Illinois to serve as member of a multi-disciplinary treatment team and performs extensive casework in the role of Treatment Coordinator.  Provides comprehensive social work services for the unit including formulation of psychosocial assessments; designing, developing, implementing, and evaluating treatment plans; providing individual, group, and family therapies; and develops discharge plans for continuing recovery in the community. Serves as a member of various Performance Improvement Teams to promote quality and safety initiatives. Develops comprehensive psychosocial assessments with information gathered through interviews. Assesses financial status of assigned patients.

Essential Functions

  • Serves as member of a multi-disciplinary treatment team and performs extensive casework in the role of Treatment Coordinator.
  • Provides individual therapy and counseling to assigned patients.
  • Develops comprehensive psychosocial assessments with information gathered through interviews with patients, families, and community referral sources. Identifies patient strengths and needs and makes recommendations for treatment and discharge planning.
  • Provides discharge planning to link individuals with appropriate services for continued recovery in the community.
  • Assesses financial status of assigned patients.
  • Serves on the facility hospital performance improvement teams.
  • Performs other duties as required or assigned which are reasonably within the scope of the duties enumerated above.
