About the job

As a key member of the clinical leadership team, the Clinical Manager supports and independently manages an interdisciplinary team, oversees the delivery of high-quality and efficient care, and ensures an exceptional patient care delivery experience with every encounter.

While this is a fully remote role, please note you will be expected to work core business hours that align with Pacific Time.

What you’ll do

  • Supervise interdisciplinary team members, including Registered Nurses, Social Workers, and Medical Assistants and perform annual reviews. 
  • Ensure adequate and efficient patient care coverage
  • Manage clinical quality initiatives
  • Support the achievement of KPIs assigned to the care delivery team
  • Collaborate with Providers and multidisciplinary professional staff to enhance patient care, implement change and develop process improvements 
  • Ability to provide leadership in the areas of conflict resolution, mentoring, skilled decision making, time management, adaptability and dedication  
  • Flex to direct patient care delivery as needed to support organizational goals
  • Development of and assist in onboarding of newly hired clinical staff (RN, SW, Provider, MA)
  • Ensure team regulatory compliance (state license renewal, certification check-offs, etc.)

Your experience and qualifications

  • Current California RN license required, with the ability to obtain additional state licensure as needed
  • 5+ years in direct care delivery role as a RN or MSW
  • 2+ years of experience in palliative care, hospice care, or complex care management
  • 2+ years of people management experience
  • Adheres consistently to the company's policies and guidelines, ensuring professional integrity.
  • Utilizes video conferencing technology effectively for remote collaboration and communication.
  • Demonstrates efficient resource management, optimizing the use of assets and tools.
  • Actively engages in fostering and nurturing positive relationships with all stakeholders and partners of the company.
  • Maintains strict confidentiality of information, fully compliant with HIPAA regulations and standards.
