Health Services of North Texas (HSNT) is currently in need of a family physician that is seeking to maximize their clinical experience by providing exceptional patient care across a broad range of integrated healthcare solutions. We “close the gap” by leveraging our vast resources to enable continuity of care, with a focus on chronic conditions and infectious diseases. We also provide excellent primary medical care for children and adults with a focus on establishing a medical home for comprehensive healthcare.

Our physician providers operate at the very highest level of their expertise as they provide comprehensive, team-based care in a fast-paced environment within a non-profit, community healthcare setting. Our team loves HSNT because we treat patients from all walks of life--regardless of socio-economic status--for a wide range of acute and chronic physical, mental, and social health conditions and in collaboration with a team of medical professionals.

HSNT is seeking a physician provider that is passionate about providing care, education, and resources in a community health setting to patients and families that have very limited options for their medical needs; and a physician provider that is ready to embrace the challenge of maximizing their already existing knowledge base and experience in chronic conditions and infectious disease, including HIV.

A day in the life of our physician provider may look like this:

• Provide professional medical services as a primary care physician.
• Assure continuity of care by providing comprehensive medical services within the scope of his/her training and capability.
• Review initial intake, health history, and laboratory test results, performs physical examinations and designs treatment plans catered to patient’s health status to provide primary care, preventative health care and health maintenance.
• Provides additional assessments and evaluations of medication therapies and treatment regimen during routine follow up clinic visits, ensuring patients continue to receive effective, efficient primary health care.
• Refer patients as appropriate for special care services, lab and x-ray and any other service that is within the scope of the patient’s well-being.
• Educates patients on medication regimens and drug adherence.
• Monitor clinical measures and collaborate with providers to assure goals are met.
• Lead and direct the development and implementation of high quality patient care services, programs and initiatives across the broad primary care continuum, as well as in specialty care areas as appropriate to locations identified.
• Uphold and ensure Medical Center’s compliance with all medical policies, directives, rules, regulations and performance standards as defined by the state, federal government, and HSNT’s policies and procedures.
• Collaborate with primary care and specialty providers; provide clinical support and facilitate decisions on medical issues when disputes arise.
• Perform clinical supervision of medical staff, including mid-level providers.
• Meet weekly with Medical Director to review agenda and action item outcomes from the medical support team meetings.
• Travel as necessary to meet Center operational needs.
• Adheres to all Agency and Medical Center policies and procedures.
• Assumes other duties as assigned by the Medical Director.

Requirements

 We ask our provider candidates to possess a minimum of:

• Advanced medical degree.
• Experience in a clinical practice as a physician preferably, in a community based program is preferred.
• Current full unrestricted licensure to practice medicine in the State of Texas.
• Board certification in area of specialty.
• Current registration with DEA and other such certificates required to legally operate as a practitioner in State of Texas.
• Possess a strong community health/public health orientation.
• Demonstrated understanding and/or experience in working in a patient-centered medical home environment.
• Demonstrated understanding and/or experience working in a integrated delivery environment.
• Excellent written and verbal communication skills.
• Strong problem solver with demonstrated analytical skills.
• Working knowledge of comprehensive health care issues. Experience or education in HIV and/or infectious diseases strongly preferred.
