RN Care Coordinator - FT - Days (72982)

Shift: Monday - Thursday 8 am - 5 pm
Friday 8 am - 12 pm

JOB SUMMARY

The Care Coordinator integrates care and services of a select high risk patient population across the continuum of care. He/She works collaboratively with patients, families, physicians, and other members of the health care team to achieve the highest quality clinical outcomes with the most effective use of available resources. The Care Coordinator assumes responsibility for an interdisciplinary process which assesses, plans, implements, monitors and measures the effectiveness of interventions to meet patients treatment and transitional needs. The Care Coordinator is knowledgeable about various age related care protocols and functions with the scope of their practice. He/She will serve as the contact point, advocate and informational resource for the patients under his/her care. The care coordinator will also work with members of the wellness program to coordinate preventive care.

JOB QUALIFICATIONS

EDUCATION: Graduate of an accredited Registered Nurse (RN) program

Licensure: . Registered RN License ; .AHA Healthcare Provider BLS or ARC

EXPERIENCE: .Preferred experience in a clinical or hospital setting. Previous experience in care coordination or case management preferred.

JOB SKILLS: Knowledge of clinical patient care. The ability to use critical thinking skills and Highly organized with the ability to maintain accurate documentation and reporting. Demonstrates evidence of exceptional leadership, communication and collaboration with other health care team members. Maintains core values that are consistent with patient/family- centered care.
