Summary:

The Clinical Outcomes Supervisor is responsible for the day to day oversight of all member-focused clinical outreach efforts at Cook Children’s Health Plan with a strong emphasis on telephonic call center oversight, preventative care, member screening, and member satisfaction. This position will provide leadership and guidance to the Health Plan Clinical Outreach team and will serve as a liaison to Quality Improvement, Service Management and Service Coordination. The supervisor is also responsible for coaching, resolving issues and serving as a link between subordinates and upper management. The ideal candidate will be a competent individual who will be able to guide and train employees. He/she will be well-versed in processes under the role’s responsibility and will be results-driven and focused.

Education & Experience:

  • High School Graduate required.

  • At least 6 months of supervisory experience. 

Licensure, Registration, and/or Certification:

  • Certified Community Health Worker OR Licensed Bachelor of Social Work required.
