Requirements

Requirements:

• Current Registered Nurse (RN) License in the state of Michigan
• Minimum of 1 year experience in hospice or home health care
• Strong assessment and communication skills
• Ability to work collaboratively in a team-oriented environment
• Valid driver's license and reliable transportation

Key Responsibilities:

• Conduct thorough assessments of patients' needs and develop individualized care plans
• Oversee the delivery of care provided by nursing assistants and other team members
• Document patient progress and updates accurately and timely
• Provide emotional support and education to patients and their families
• Participate in interdisciplinary team meetings and contribute to patient care strategies
