Job Summary

Great opportunity to join an experienced Case Management team. Role focuses on transition planning for our high risk population

SW Case Managers are assigned geographically to inpatient units and work on consult basis

Support staff includes a Community Health worker to connect clients to community services

Looking for good critical thinking skills and willingness to learn in a fast paced environment.


 

Qualifications

Job Specific Responsibilities:
  • Initial assessment and development of transition plan 
  • Coordination with a multidisciplinary team  
  • Patient advocacy 
  • Ability to delegate tasks to  Community Health  Worker

Qualifications Required:
  • High degree of skill in written/oral communications and personal interactions.  Demonstrates agility in a continuously changing fast paced work environment.  Demonstrates computer competency. Ability to complete daily job responsibilities within normal hours by moving rapidly through the hospital and physician offices.  Must be able to perform the essential job functions, either unaided or with the assistance of a reasonable accommodation to be determined by the organization on a case-by-case basis.  Culturally competent.  
  • Three or more years of clinical experience in a health care delivery system.
  • Completion of an accredited  masters level SW program.
  • Current SW  License in the State of MA is required.
Preferred:
  • Case Management, Home Care Discharge Planning experience
