Description
Hospice of the Piedmont invites you to join our compassionate team as a Hospice LPN (PRN), providing professional nursing services to patients and their families within the Hospice House. Your role will be crucial in delivering high-quality end-of-life care. You will supervise and collaborate with housekeeping, Home Health Aides (HHAs), and volunteers during your shift.
Responsibilities/General Duties (including but not limited to):
- Provide direct nursing care to hospice residents and oversee nursing assistants.
- Collaborate as a team member to identify and address patient care needs.
- Report changes in the resident's condition to the RN Case Manager, Team Leader, or Physician
- Document patient information accurately and with sensitivity.
- Assist with food preparation for residents when needed, promoting a homely atmosphere.
- Maintain housekeeping standards to create a comfortable and dignified living environment.
- Ensure compliance with health, fire, and safety measures to uphold the well-being of residents.
- Utilize universal precautions for infection control, emphasizing a safe and nurturing space.
- Provide guidance and training to house staff with a focus on compassionate care.
- Support performance evaluation processes and provide coaching to enhance individual performance.
- Adhere to personnel policies and procedures, fostering a culture of respect and support.
- Take responsibility for medication management and documentation, ensuring the highest standard of safety and care.
Requirements
- High School Graduate or GED.
- Graduation from an accredited school of practical nursing.
- Licensed as an LPN in the Commonwealth of Virginia.
- Two years of relevant nursing experience.
- Ability to manage terminal care, coordinate health team services, and maintain accurate documentation.
- Nursing home/assisted living facility experience preferred.
- Basic knowledge of Medicare/Medicaid reimbursement regulations.
- Emotionally mature and compassionate individual.
- Basic to intermediate proficiency in word processing software.
- Effective communication and interpersonal skills.
- Ability to work in a team and maintain confidentiality.
- Flexibility to work beyond scheduled hours when necessary.
- Basic computer skills required.
