Position Summary:
We are looking for an enthusiastic Activities Assistant to support the planning and execution of engaging individual and group activities. This role is essential in fostering communication among residents, families, staff, and external partners, ensuring that the needs and interests of our residents are met.
Key Responsibilities:
- Activity Planning: Participate in the development and facilitation of a diverse range of individual, small, and large group activities that cater to the interests of residents.
- Communication Facilitation: Ensure effective communication among staff, residents, families, support personnel, and external agencies to meet the needs of our community.
- Community Engagement: Assist in community planning efforts to align facility services with resident and family needs.
- Activity Calendar Management: Help develop the monthly activity calendar and maintain accurate attendance records.
- Quality Assurance Support: Collaborate with the Quality Assurance and Assessment Committee to implement corrective action plans as directed by the Activities Director.
- Resident Assessments: Contribute to discharge planning and the creation of individualized activity care plans based on resident assessments.
- Transportation Coordination: Arrange transportation for residents for outings, appointments, or discharges as needed.
- Inclusive Activities: Design a monthly activity schedule that includes outings, group activities, and in-room options for residents with mobility challenges.
- Encouragement of Self-Directed Activities: Promote participation in hobbies, crafts, and reading, providing materials in accessible formats such as Braille or audio books.
- Documentation Support: Assist with assessment documentation and quarterly progress notes as assigned.
- Department Maintenance: Keep the Activities Department clean, organized, and secure.
Qualifications:
- Education & Experience: High school diploma or equivalent; one year of experience in a long-term care facility is preferred.
- Language Skills: Ability to read and comprehend technical procedures, policy manuals, and effectively communicate with staff and management.
- Mathematical Skills: Capable of applying concepts such as fractions and percentages to practical situations.
- Reasoning Ability: Strong problem-solving skills and the ability to interpret various forms of instructions.
