Licensed Transition of Care Specialist, VA Foster Care (North Region)

Job Description:

LOCATION: This is a virtual eligible position for the Virginia Foster Care program. The ideal candidate will reside in the North Region of Virginia.

HOURS: General business hours, Monday through Friday.

TRAVEL: Some travel (member/provider facility) within the Virginia North region may be required.

This position is sign-on bonus eligible!!

This role enables associates to work virtually full-time, with the exception of required in-person training sessions, providing maximum flexibility and autonomy. This approach promotes productivity, supports work-life integration, and ensures essential face-to-face onboarding and skill development.

The Transition of Care Specialist (Special Programs Case Mgr I) works directly with members, their families and providers/facilities to evaluate member needs, coordinate care and develop successful discharge plans during transitions from inpatient settings, between levels of care, and during transfers from residential settings.

Primary duties may include, but are not limited to:

  • Conducts assessments to identify individual needs.

  • Develops comprehensive care plan to address objectives and goals as identified during assessment.

  • Supports member access to appropriate quality and cost effective care and modifies plan(s) as needed.

  • Coordinates with internal and external resources to meet identified needs of the member in terms of integrated (physical and behavioral) whole person care.

  • Coordinates social determinants of health to meet the needs of the member and incorporates that into care planning.

  • Works closely with various state agencies.

  • Maintains knowledge of the system of care philosophy; a spectrum of effective, community-based services and supports for those with or at risk for mental health or other challenges and their families, that is organized into a coordinated network.

  • Builds meaningful partnerships with designated populations and their families, and addresses cultural and linguistic needs, in order to help them function better at home, in the community, and throughout life.

  • Evaluates health needs and identifies applicable services and resources in conjunction with members and their families.

  • Provides important information including patient education, medication reconciliation, and identification of community resources and assists with arrangement of follow-up care.

Required Qualifications

  • Requires an active, current and valid license such as an LCSW, LMSW, LPC, LAPC, LMFT LMHC, or RN issued by the Commonwealth of Virginia.

  • Requires MS/MA in social work, counseling, or a related behavioral health field or a degree in nursing.

  • A minimum of 3 years of clinical experience in social work counseling with broad range of experience with complex psychiatric and substance abuse or substance abuse disorder treatment; or any combination of education and experience, which would provide an equivalent background is required.

Preferred Qualifications

  • Experience with Transition of Care including discharge planning, and wrap-around services for youth leaving inpatient, physical and/or behavioral health settings is highly preferred.

  • Knowledge of the Virginia's Foster Care and child welfare system is extremely helpful for this role.

  • Case management/discharge planning with a broad range of complex psychiatric/substance abuse and/or medical disorders is very strongly preferred.

  • You will need very strong communication skills in order to be successful in this role.

  • You must be computer literate and have some experience using Microsoft applications (Word, Excel, Outlook), etc.
