Linwood Meadows Care Center is seeking a compassionate and motivated Certified Occupational Therapy Assistant (COTA) to join our dedicated rehab team. Key Responsibilities:

• Provide occupational therapy treatments under the direction of a licensed Occupational Therapist (OT)

• Deliver therapeutic interventions to help residents achieve functional goals

• Document daily treatments, progress notes, and updates in compliance with state and facility guidelines

• Assist in care planning and discharge preparation

• Educate residents and caregivers on techniques for improved mobility and daily living activities

• Collaborate with interdisciplinary team members including nursing, PT, and SLP

• Maintain a clean and safe treatment environment

• Participate in in-services, trainings, and facility quality improvement initiatives  Qualifications:

• Active California COTA license in good standing

• Graduate of an accredited Occupational Therapy Assistant program

• CPR certification (or ability to obtain upon hire)

• Experience in skilled nursing or geriatric rehab preferred

• Strong communication, time management, and documentation skills

• EMR proficiency (experience with Casamba or similar systems a plus)
