Physical Therapist

St. Barnabas is hiring for a part-time Physical Therapist!

Essential Duties as Physical Therapist:

• Directs evaluation and development of the rehabilitative plan of care and in periodic pre-evaluation as indicated
• Assists in teaching exercise programs to client and family
• Consults with the rehab team regarding the outcome of home visits and reports physical findings
• Writes daily progress notes on client and submits daily
• Confirms on a weekly basis the scheduling of visits

Plans, implements and evaluates patient/client care plans to restore or maintain patient/client well-being.

Plans, organizes and conducts physical therapy treatment program based on the medical referral and their evaluation. Participates in the coordination of care.

Develops and evaluates the plan of care in partnership with the patient/client

Plans and provides appropriate treatment for neuro-muscular, neurological and orthopedic disorders based on medical referral and therapy assessment.

Job Qualifications:
Education:

Bachelor of Science in Physical Therapy from an American Physical Therapy Association approved program.

Licensure:

Current State License Current State License or a multi-state license issued by a state member of the Physical

Experience:

One year experience as a Physical Therapist in an acute care or rehabilitation setting or home health preferred.

Must be able to lift 50 lbs
