About the role

An Integrated Mental Health Therapist at Westfields Hospital & Clinic works with patients, families, care givers, and health care team members in partnership with the community to help patients achieve optimal health goals. This person will provide patients with support, education, referrals, and resources through comprehensive, coordinated, patient and family-centered care.  The Integrated Therapist provides culturally appropriate, non-judgmental, compassionate care while maintaining professional accountability and engaging in continuous quality improvement.

  • Comprehensive Clinical Assessments.
    • Ensures that all interventions are performed and documented according to professional clinical standards.
    • Completes clinical assessment including patient’s social history, social resources, financial resources, environment, self-care capacity, mental/emotional status, strengths, and weaknesses.
    • Facilitates establishment of plan to meet client’s needs with patient/family and available community resources.  Makes appropriate referrals.
    • Assists family in accessing available resources.
  • Information and referral for community resources to patients and families.
  • Establishes and maintains resource file by county.
    • Establishes contacts at county and community agencies to maintain updated information on programs.
    • Maintains up-to-date knowledge of financial resources in the community for assisting with complex adult cases.
    • Assesses patient/family needs and facilitates connection with appropriate community resources.
  • Short-term, brief-intervention counseling, or pre-therapy for complex cases.
    • Assists patient and family members in identifying psychosocial issues/impacts or their situation, facilitates connection with appropriate community resources.
    • If patient/family situation dictates ongoing therapy needs (more than 1-4 visits) refer to appropriate community resource.

Work Schedule:

  • .5 FTE/20 hours weekly - Days

Required Qualifications:

  • Master's degree in social work from a CSWE accredited program and State of Wisconsin licensure (LCSW) or license eligible.
  • Clinical Social Work experience with individuals and families.
  • Working knowledge of, and experience with, family systems theory, individual and family counseling, and trauma-informed care practices.

Preferred Qualifications:

  • Experience in EPIC computer system
