Your Role as a Behavioral Health Clinical Team Manager

As a key leader, you’ll ensure seamless, high-quality home health services while building and inspiring a team of compassionate healthcare providers. You will:

• Oversee patient care services, including staffing, care coordination, and referrals.

• Ensure patient care starts promptly, following regulatory guidelines.

• Supervise and support your team to meet patients’ evolving needs through personalized care plans.

• Coordinate and communicate effectively with patients, families, physicians, and other care providers.

• Monitor clinical outcomes, update care plans, and ensure compliance with physician orders.

• Drive quality improvement and ensure your team meets or exceeds clinical standards.

What You Bring to Elara Caring

We’re looking for leaders who are passionate about making a difference. To succeed in this role, you’ll need:

• Associate’s Degree in Nursing (BSN preferred).

• 2+ years of home health or behavioral health experience as a Registered Nurse.

• 1+ year of supervisory experience in a healthcare setting.

• Active state Registered Nurse license.

• Proficiency in enterprise systems like Workday and HomeCare HomeBase, and familiarity with MS Office Suite (Word, Excel, Outlook).
