Job Summary: Responsible for the oversight of Personal Care staff and the management of the community services provided by Personal Care staff in the center, the Participant’s residence or in any other environment the Participant might be at the moment of needing care ( i.e. temporary living in another residence, SNF, hospital etc.) Serves as a home care liaison.


Essential Functions:

  • Identifies, plans, develops, organizes, implements and evaluates all Personal Care services provided by LIFE Pittsburgh and ensures that established standards, regulations and PACE philosophies are observed. Will be responsible to fulfill the requirements of the grievance and appeal program as described in procedure.
  • Participates collaboratively with Director of Clinical Services in the development, update and maintenance of policies and procedures of Personal Care Services and center operations. Demonstrate an in depth understanding of PCA protocols, clinical protocols and competency standards in order to ensure quality service delivery at all times. 
  • Actively participates in the annual budgeting process for Personal Care Services and provides ongoing monitoring and correction of budget variances.
  • Supervises and monitors the coordination of the scheduling of personal care services by meeting staffing levels for safe provision of care on a 24-hour coverage basis. Establishes and maintains cooperative working relationship among all staff, especially with Center Administrator, Nurse Manager, PCAs and other scheduling staff. Ensure that needs are appropriately met in participant home, center or other care settings.
  • As a PACE core team member, participate actively in the assessment of participants home care needs and the ongoing evaluation of needs in response the changes in participant status. In collaboration with Community Care Coordinators, Community Care Manager and the Interdisciplinary Team oversees, reviews, and monitors PCA services included in the participant's plan of care, to include changes and necessary adjustments to ensure that proper Personal Care Services are provided. 
  • Actively participate in family meetings, care plan meetings, team meetings and other participant related communications to present and explain home care services in place and help with collaborative problem-solving where needed.
  • Directs and evaluates work performance of Scheduling Associate, Personal Care Assistants and Community Care Coordinators, insuring the continuous provision of safe and quality care within the context of a PACE model. Provides periodic and annual evaluations of employee performance.
  • Provide direction and oversight of the time and attendance activities for the Scheduling Associate, Personal Care and Community Care Coordinator staff to include expense report review and time sheet monitoring.
  • Actively participates in the hiring of new home care staff as well as the orientation and training of Personal Care Assistants/ CCC/PCA schedulers during the new hire period and during annual competency reviews. 
  • Responsible for determining and maintaining appropriate levels of supplies for the Day Health Center and the Participant home needs working within expected need via the Participant Plan of Care. 
  • Other duties as assigned to meet the goals and objectives of the LIFE Pittsburgh program.
  • Provides services promptly, exercises tact, patience and courtesy at all times and conducts themselves in a professional manner with Participants, families, co-workers and any external contacts. As a representative of LIFE Pittsburgh, promotes goodwill and confidence in our Participants, staff, vendors, visitors and the general public.    
  • Proactively maintains requirements for the position (e.g., access of a vehicle, valid driver’s license and auto insurance, annual TB, bi-annual physical, etc) and maintains compliance with educational requirements (e.g., Fire Safety, Infection Control, Annual Training, Safe Mobility).


Working Conditions:

  • Works within the LIFE Pittsburgh Adult Day Health Centers (ADHC), frequently travels to and works in Participant residences and may travel between LIFE Pittsburgh sites during all weather conditions. Occasional travel for other related functions.  
  •  Awareness that working with LIFE Pittsburgh Participants (in the home or the center) involves the potential for exposure to unpleasant sights and smells, exposure to blood borne pathogens and/or other communicable diseases. Universal Precautions must be utilized at all times.
