Jefferson Healthcare is seeking a dedicated fulltime Medical Social Worker to join our Home Health team. In this role, you will deliver short-term services to support patients and families in managing personal, financial, psychosocial, and environmental challenges related to the patient's medical or psychiatric conditions by connecting them with valuable community resources. What you'll need:

• Washington State Licensed Clinical Independent Social Worker,
• Two (2) years recent LCISW experience in health care setting
• Minimum two years Social Worker experience in healthcare setting, clinical social work in similar setting.
• Experience in working with specific patient populations that may include but are not limited to: children, frail elderly, vulnerable adults, and patients impacted by mental illness
• Previous experience in AIMS collaborative care primary care clinic is a plus.
• May require valid Washington state driver's license.

Schedule: 1.0 FTE; 40 hours/week; Day Shift
