SOCIAL SERVICE DESIGNEE
Job Description: Responsible for promoting self-determination and quality of life in older adults. Responsible for networking services, documentation and providing family support for Healthcare residents.
Essential Functions:
Interview resident and family upon admission to finalize all admission paperwork. Assess resident/family needs for appropriate interventions.
Complete prior level, social history, and brief trauma questionnaire of each admission.
Discuss advance directives, support choices made by the resident and keep an annual update of medical directives of all residents.
Complete comprehensive assessment of each resident and update quarterly.
Develop and maintain plan of care with established problems, goals and approaches on each resident.
Accurate and timely completion of MDS assessments, and progress notes.
Facilitate Resident Care Conference in order to have input into development and review of the resident’s care plan, including the discharge planning.
Assist resident in maintaining their personal rights.
Establish and update discharge plans on all residents and coordinate services prior to discharge.
Provide referrals to community agencies when necessary or appropriate, for the resident’s welfare. Document and monitor behavioral concerns of residents and implement effective interventions.
Maintain a current file of social service agencies and available services.
Observe roommate compatibility, problem-solve conflicts and arrange room transfers as appropriate. Work with other disciplines to meet the needs of the residents.
Facilitate behavior team meeting and document acting plans for behavior management.
Knowledge of pre-admission screening process, complete necessary paperwork and track Level II requirements.
Assure pre-admission screening process, is completed for each resident to meet PASRR level 2 and LDC requirements.
Assist staff in completing PASRR assessments online and maintain current PASRR census.
Assisting the admissions department with collecting information, ensuring accurate and timely completion of necessary paperwork as well as assessments and notifications.
Assisting admissions with various wait list management (AL/skilled/memory care) and follow up activities related to such as phone calls, and filing of applications and reports when needed.
Cover for admissions and for Social Services Director when needed.
Assure pre-admission process is completed for each resident to meet PASRR requirements, that PASRR’s are received and provided to medical records, and maintain Ascend/Path tracker census with admissions and discharges.
Position Requirements:
1.Associate or bachelor's degree in social work or a human services field including but not limited to sociology, special education, rehabilitation counseling, and psychology; or equivalent experience required.
2. Or a certificate of completion of a Social Service Designee course.
3. Previous experience working with older adults. Nursing Home experience preferred.
4. Familiarity with Medicare, Medicaid, VA, and pre-admission screening process, or willingness to learn quickly.
