NurseCore is seeking a Clinical Director to lead home care services in Syracuse, NY. This full-time role combines clinical expertise and leadership to ensure safe, high-quality patient care in compliance with New York state and federal regulations.
Responsibilities:
- Supervise nursing staff and Home Health Aides, ensuring training and competency
- Manage client care: assessments, care plans, recertifications, coordination, and discharge planning
- Oversee referrals, staffing, recruitment, onboarding, and scheduling
- Conduct home visits, clinical reviews, and PAG meetings
- Address client concerns and maintain accurate documentation
- Active RN license in New York
- 2+ years RN experience, including 1+ year in home health
- Leadership experience supervising staff, including hiring/firing
- BSN or 4-year degree preferred
- Strong problem-solving and communication skills
- Full-time, Monday–Friday, 8 a.m.–5 p.m.; occasional evenings/weekends
- Local travel; occasional out-of-area/overnight travel
