Job Description
PRIMARY RESPONSIBILITIES
Caregiver for individuals in the agency.
CHARACTERISTIC DUTIES
§ Assist the individual in his/her as documented in the ISP, Service Plan etc.
§ Routine hygiene care throughout the day.
§ Medication reminder
§ Supervise in addressing the ISP/Plan of Care goals and document progress
§ Support all opportunities for the growth, learning and development of the individual
§ Complete daily progress notes highlighting person center events
§ Complete understanding of home policies and procedures including home rules, disaster procedures, participant’s rights, etc.
§ Document progress notes before leaving the job site
§ Document tracking of ISP goals prior to leaving the job site
§ Ensure the individual is integrated into the community
§ Notify the agency of any concerns regarding the individual
§ Attend all staff meeting (required), participate in scheduled training sessions including on the job training
§ Promote harmonious relationship with other staff members
§ Maintain individual’s records on site in accordance with policy and procedures
§ Other duties as assigned
Qualifications
MUST HAVE THE FOLLOWING:
CPI
CLEAN 7 YEAR MVR
NATIONAL BACKGROUND CHECK (ONE TIME $60 FEE)
RESUME (MUST REFLECT EXPERIENCE WITH DBHDD CLIENTS)
CURRENT PHYSICAL
CURRENT TB TEST
CERTIFICATES OF TRAINING
CURRENT DRIVERS LICENSE
AUTO INSURANCE
COPY OF PROFESSIONAL LICENSE
COPY OF SOCIAL SECURITY CARD
FIRST AID CERTIFIED(required)
CPR CERTIFIED (required)
