DaVita is pioneering a new approach to primary care for patients with end-stage renal disease (ESKD) directly within our dialysis centers. We're seeking a highly motivated and innovative Medical Assistant to be our founding MA, helping us build and shape this groundbreaking program.
It is estimated that 30-50% of patients with renal failure do not receive consistent primary care support, resulting in increased morbidity, hospitalizations, and mortality. You will work with a Nurse Practitioner to address the health barriers patients with kidney disease face. Our goal is simple: keep patients out of the hospital and living longer. Come join us!

Location & Travel:
- Commute to DaVita clinics and/or partner practices in the Norfolk/Virginia Beach area and surrounding areas 5 days a week.

Position Details:
- Full-Time, Monday–Friday schedule
- Be flexible and adaptable to respond to patient and program needs
Responsibilities:
- Coordinate care for patients by ensuring access to eligible resources/programs and assisting with referrals and provider appointments
- Perform outbound calls and respond to inbound calls from patients, providers, and Village resources
- Educate patients about the benefits of this program, and then enroll and onboard the patients (such as collecting medical information and creating a record in our Epic platform)
- Complete non-clinical screenings and pathways under the guidance of the Manager; escalate to Integrated Care Nurse as needed
- Assist the clinical team with moving patients through the continuum of care by completing tasks as directed
- Manage clinical data so that key clinical reports can be produced. This includes transferring information from outside medical records and auditing data for clinical integrity.
- Communicate directly with patients in a timely and professional manner
- Maintain accurate, complete, and confidential documentation
- Demonstrate exceptional two-way communication, both oral and written
- Act in alignment with DaVita’s mission and values to deliver high-quality patient care
Qualifications:
- High school diploma or GED required
- Hold a current national Medical Assistant certification such as CMA, RMA, or CCMA
- 2+ years of experience in a professional office environment
- Experience coordinating care (referrals, care management, etc.)
- Ability to handle confidential information with discretion and sound judgment
- Strong verbal communication skills
- Proficiency in Microsoft Office, Excel, and Outlook
- Commitment to service excellence and patient satisfaction
- Ability to work autonomously while collaborating effectively with the team
Preferred Qualifications:
- 4+ years clinical medical office experience
- Experience in primary care settings
- Experience in dialysis settings
- Experience with Epic EHR
- Familiarity navigating insurance payers
