About the role

Job Description:

The LGBTQ+ Social Work Navigator is a patient-focused role that helps successfully manage the comprehensive care of patients. This position provides clinical assessment and develops plans of care with patients, helps coordinates and connect to needed services and resources. The Social Worker helps assist to close care gaps, supporting patients to meet their goals, coordinating resources to help patients overcome socioeconomic barriers, and resolving patient issues when possible. This includes receiving, prioritizing, documenting, and actively resolving requests from providers, clinics, and patients. This position reports to the practice manager and works collaboratively with primary care, specialties and service lines.

Job Essentials:

  • Coordinates the care and services for patients identified as needing assistance. Collects information regarding a patient’s medical and psychosocial condition, to identify individual needs and develop a plan of care. Works with patient and family/caregivers to determine specific goals and actions based on assessment.
  • Educates patients about available support services, as well as community resources, and helps guide patient on realistic expectations regarding support
  • Collaborates with primary and specialty disciplines to support the patient's plan of care, including but not limited to assuring access to services, providing individualized support and reinforcement of education, and connecting patients and families with available community resources, as needed.
  • May act as a liaison with other caregiver groups, clinics, and departments
  • Oversee and help with Gender Board, in collaboration with the Care Coordinator
  • Provide short-term therapy, which may include providing WPATH Letter of Assessments for gender affirming surgeries
  • Develops a plan of care either individually or with input from the patient, patient family and significant others, physicians, and other members of the healthcare team.
  • Acts as an advocate on behalf of patients and families, where appropriate, to ensure effective communication with multidisciplinary care team, outside agencies and other resources as necessary.
  • Complies with federal, state, local, and clinical program/facility standards.


Minimum Qualifications

  • Master's Degree in Social Work. Education must be obtained from an accredited institution. Degree will be verified.
  • Current State Licensure as LCSW or LMFT.
  • Computer Skills, i.e., email, word processing.
  • Excellent written and verbal communication skills.

Preferred Qualifications

  • LGBTQ+ community experience, including an understanding of biopsychosocial issues, as well as LGBTQ+ health issues
  • Experience working with other marginalized communities
  • Competency in electronic patient charting
  • Experience in healthcare disciplines
  • A working knowledge of the healthcare industry, roles, and terminology

Physical Requirements:

Ongoing need for employee to see and read information, labels, assess patient needs, operate monitors, identify equipment and supplies.

Frequent interactions with patient care providers, patients, and visitors that require employee to verbally communicate as well as hear and understand spoken information, alarms, needs, and issues quickly and accurately, particularly during emergency situations.

Manual dexterity of hands and fingers to manipulate complex and delicate equipment with precision and accuracy. This includes frequent computer use and typing for documenting patient care, accessing needed information, medication preparation, etc.

Need to walk and assist with transporting/ambulating patients and obtaining and distributing supplies and equipment. This includes pushing/pulling gurneys and portable equipment, including heavy items. Often required to navigate crowded and busy rooms (full of equipment, power cords on the floor, etc).

May be expected to stand in a stationary position for an extended period of time.
