JOB SUMMARY:
Provides Outpatient addiction counseling and general outpatient services to a treatment court population. Assure adherence to state statutes and rules, CARF standards, and federal regulations including HIPPA and 42 CFR Part 2. Positively represents the organization and promotes Rimrock’s Mission to the community.
ESSENTIAL FUNCTIONS:
- Conducts outpatient therapy and/or ancillary groups as assigned for treatment court participants according to the written criteria, policies and procedures. This includes individual, group and family therapy.
- Prepare timely and accurate records on patients and maintain substantial compliance on medical records audits. Assures all records meet state and CARF standards.
- Function as a member of treatment team and demonstrate ability to coordinate needed services for patients, and to communicate with supervisor for court purposes, as well as providing input on client progress.
- Complete weekly court reports and maintain records of progress for court.
- Complete assigned court assessment in a timely manner and recommend appropriate placement.
- Provides assessments on assigned clients using appropriate testing instruments and biopsychosocial. Applies patient placement criteria to assure appropriate referral of patients within the continuum of care.
- Works with the Clinical Supervisor to interpret the biopsychosocial database for the purpose of identifying and prioritizing the clinical problems to be addressed.
- Develops and implements the patients’ Master Individualized Treatment plans.
- Coordinates and monitors with assistance from supervisor, the appropriateness and flow of all treatment services for assigned patients.
- Provide educational lectures and/or ancillary services for Outpatient clients.
- Provides discharge planning services and assures continuing care plans for patients at the appropriate level of care.
- Attend all scheduled staff meetings.
- Provides continuing care services and coping skills curriculum for assigned cases
- Other duties as assigned.

EDUCATION/TRAINING:
Baccalaureate degree in Human Services or
