Job Description:

Under the leadership of the department director and assistant director the Patient Care Coordinator is responsible for the assessment of post-hospital care needs and appropriate continuing care for patients upon discharge. S/he possesses the skills and knowledge to assist patients’ family members, significant others, physicians, and other health care providers in assessing patients’ continued care needs, and provides information on appropriate options, community resources, and insurance coverage to meet those needs. S/he will assist the interdisciplinary team with recommendations on the appropriateness of admissions and discharge and assist with ordering durable medical equipment for discharged patients. S/he also provides administrative support.

Important dimensions of this job include interpersonal communication and public relations skills. Must be able to develop and maintain a professional, service-oriented working relationship with patients, physicians, coworkers and supervisors. Strong team player and ability to maintain a positive professional demeanor with the community. Excellent customer service skills.

Experience:

S/he must have worked in health care-related field with the knowledge and understanding to review patient diagnoses, treatment plans, and outcomes. Preferences include: experience in an acute care hospital setting,

knowledge of Medicare and other financial reimbursement processes for hospital services. Spanish speaking preferred.

Education:
Bachelor’s degree in health care field preferred.

Day shift - 8:30 a.m. - 5:00 p.m., Sun-Thurs

Pay Range (based on years of applicable experience):

$40.96

to

$54.78

The hours employees work determine when a shift differential is paid.

Hourly Evening Shift Differential: $2.99

Hourly Night Shift Differential: $4.48
