Description:

The Community Partnership Specialist will manage, coordinate and participate in the development of community resources in partnership with both public and private agencies within the community. This position is responsible for the establishment of viable coalitions and partnerships among civic, community and health and human services partners to plan programs, services and supports designed to meet the needs of families with newborn children. The Community Partnership Specialist will solicit feedback from home visitation team members regarding unmet needs in the community and, with the team and community partners, plan toward closing those gaps.

SCHEDULE:

This position is typically flexible, allowing for a 4-day work week (Monday-Thursday) or a 5-day work week (Monday-Friday). This position allows for hybrid work with on-site and community-based work required, in addition to remote work, depending on business need.

COMPENSATION:

Hiring Range: $29.29 – $32.54 USD Hourly

Compensation will be determined based on education, experience and skills.

ESSENTIAL DUTIES:

  • Establish and nurture relationships, partnerships, and coalitions with community resources and organizations that provide support to families in a timely and appropriate manner, as referred to by Family Connects nurses, while actively participating in the community system of care for young children and their families.
  • Create feedback loops with community partners to encourage transparency and follow through.
  • Lead and coordinate the Family Connects Community Advisory Board.
  • Lead and manage the Family Ambassador Space, including recruiting and building relationships with families, planning for and facilitating Family Ambassador meetings, tracking family engagement and participation.
  •  Identify and build relationships with service providers, maintain an updated resource list through the Agency Finder directory, and identify gaps in community support to encourage the broader community to expand services over time.
  • Provide individual-case consultation about community resources during nurse case conference and as needed to support nurse home visitors as they connect families to resources. Encourage bi-directional communication with nursing staff for continuous quality improvement with regards to community resources, referral processes and needs assessments.
  • Collaborate with diverse groups, including community members, organizations, and networks, to understand cultural expectations and identified needs.
  • Other duties as assigned.

Minimum Qualifications:

  • Bachelor's degree in Public Health, Sociology, Communications, Public Administration or a related field
  • 3 years of work-related experience
  • Note: An equivalent combination of education and experience is acceptable

Preferred Qualifications:

  • Experience working in government, public health, or non-profit sectors
  • Experience working with diverse organizations in community settings
  • Bilingual in Spanish/English highly preferred
