Classification Minimum Requirements:

Bachelor's degree in in an appropriate; or an equivalent combination of education and experience.

Job Description: Assists in managing one or more clients in PrEP education, enrollment, and follow-ups, whether in-person at the clinic, on the mobile testing unit, at their homes, via telephone or virtual encounters.   Maintains an active caseload providing interventions as needed and within area of expertise and limits of credentials. Works with the client on a regular basis using professional judgment and discretion to address barriers to clients’ enrollment and adherence to PrEP. Supports clients in navigating the clinic system and community resources.   Collects, enters and stores clients’ hard copy data and any related personal and health protected information into designated and secure lockboxes, or cabinets based on guidelines. Additionally, Collects, enters, and stores clients’ electronic data and any related personal and health protected information in designated and secure Excel spreadsheets as well as in appropriate and secure electronic medical records software   Maintains liaison with other outreach staff by providing HIV and STD counseling, education, testing and available for outreach prevention services in the community. This includes assisting with the marketing and promotion of outreach services to the population of focus and providing outreach services to clients in area of clinical specialty. May include operation/driving of UF CARES Mobile Unit. Attends meetings as appropriate and meets regularly with supervisor to exchange pertinent information.   Maintains liaison with clinical staff (Clinical Service Representatives, Medical Assistants, Registered Nurse) and Providers to facilitate linkage to PrEP clinical services.    Maintains liaison with the 340B Program Coordinator to assist insured clients with the enrollment process   Documents and collects clients’ information and refers to appropriate agencies for healthcare marketplace insurance enrollment assistance and/or linkage to appropriate services internally or externally based on clients’ needs   Completes all trainings and ongoing certifications as required. Participates in continuing education activities, remaining knowledgeable in areas(s) of expertise.   All other duties as assigned. Expected Salary:

$43,888 to $55,000 annually depending on education and experience

Required Qualifications:

Bachelor's degree in in an appropriate; or an equivalent combination of education and experience.

Valid driver's licene required.

Preferred:

Bachelor’s degree with experience in Health Education and knowledgeable about PrEP.
