Responsibilities

The Care Coordinator (CC) is responsible for supporting ECM patients through care coordination and linkage to services and social supports.  The Care Coordinator (CC) will be responsible for appointment scheduling, referral management for ECM patients and will work closely with the ECM team in developing care plans to address medical, behavioral, and social needs as well as monitor and document progress and adjust care plans as needed. The CC will assist with departmental goals, reports and communicate with executive leadership. 

Qualifications

EDUCATION, EXPERIENCE, TRAINING

 

1.            Utilization Review experience preferred.

2.            BA Degree preferred.

3.            Excellent written and verbal communication skills. Excellent critical thinking skills.

4.            Excellent interpersonal skills to build effective partnering relationships with physicians, nurse staff, coding staff and hospital management staff.

5.            Ability to work independently in a time-oriented environment.

A reasonable compensation estimate for this role, which includes estimated wages, benefits, and other forms of compensation, is $25.00 to $33.65. The exact starting compensation to be offered will be determined at the time of selecting an applicant for hire, in which a wide range of factors will be considered, including but not limited to, skillset, years of applicable experience, education, credentials and licensure.
