Licensed Social Worker- Case Management-Full Time

A Non-Clinical Licensed Social Worker in this capacity is a social worker who initiates and completes case management duties and discharge planning. As a Non-Clinical Licensed Social Worker, in the acute inpatient hospital setting, you will provide post-hospital planning and resources for patients to meet their health and psychosocial needs in the least restrictive environment.   Minimum Education

• Bachelor of Social Work

Minimum Work Experience

• One year work experience in a related field.

Required Licenses

• Registered with Kentucky Board of Social Work.
• If not licensed, you will have 6 months from date of hire to obtain the licensure required for the position
• Basic Life Support (BLS) or Cardiopulmonary Resuscitation (CPR) certification within 3 months of hire date.
