About our Team: 

We employ a field-based, home-based care model and are committed to meeting members where they are--in their homes, in their community, and in our Hubs. You will go above and beyond to connect with Cityblock members in a non-judgmental, respectful and empathic manner, to meet their needs, and to provide feedback to the system as a whole as we strive to do better every day.

About the Role:

As a Perinatal Community Health Partner, you will work with a growing panel of members on improving their health holistically before, during, and following pregnancy, regardless of how that pregnancy ends.  You will build trust with members on your panel and engage with them to understand their strengths, goals, wants and needs across various aspects of their lives. The information you glean from your conversations will inform the development of our MAP (Member Action Plan). Your ongoing work with the member will be to determine the member’s priorities and work on their goals.  You will be a member’s point person, problem solving alongside them in times of crisis, and acting as a non-judgemental listening ear. You will work alongside, and will be supported by, an interdisciplinary clinical care team to ensure all member needs are met.  You will accompany your panel of members through their pregnancy care experiences, including but not limited to birth, abortion and the postpartum period.

You will work as part of an Interdisciplinary Team, including a Certified Nurse Midwife, Perinatal Care Team Lead, Behavioral Health Specialist, Perinatal Nurse Care Manager, and Perinatal Community Health Partners.

Requirements for the Role:

  • Bachelor’s degree
  • You have experience as a doula/community birth worker with training and timely certification
  • Current Doula Certification or up to date training (please specify with whom you have been trained)
  • Minimum 3 years case management or related field.
