Job Summary:

Assists with personal care and homemaking tasks to enable patients to remain in their homes in a caring and compassionate manner.  Assists patients and families by providing emotional support. Performs services as ordered by the clinical team which may include a variety of medical duties such as vital signs.  Works under direct supervision of an LPN or RN.  May only perform care within the scope of a CNA.

Supervisory Responsibilities:  None

Duties/Responsibilities:

  • Uses only approved abbreviations
  • Understands patient safety goals & actions taken
  • Uses 2 patient identifiers before giving treatments, demonstrates process
  • Follows aide care plan developed by clinical team (nurse/provider)
  • Bathing (skin, hair, nail and mouth care) & hands on personal care, assistance with ambulation or exercise, feeding & positioning
  • Assisting with light housekeeping such as meals and laundry
  • Encouraging patient independence as appropriate
  • Psychosocial, spiritual, & palliative care/support
  • Apply creams/powder/ointment to intact skin, if indicated on the plan of care
  • Convert a urinary drainage bag to a leg bag, if indicated on the plan of care
  • Nasal cannula care
  • Pediculosis or scabies treatment, if indicated on the plan of care
  • Post-mortem care
  • Prosthesis care, if indicated on the plan of care
  • Apply & remove splinting devices, if indicated on the plan of care
  • Apply & remove TED hose, if indicated on the plan of care
  • Attends case conferences as needed/IDG
  • Informs nurse/office of changes in patient’s condition & any concerns from the patient/family/caregiver
  • Schedules nurse aide visits as directed by case manager
  • Assists with scheduling conflicts as neededTriage calls and manage appropriately
  • Answer phones in a professional & customer sensitive manner
  • Accepts referrals and requests information from registration for new referrals
  • Creates new referral matrix
  • Obtain medical records as needed by clinical team
  • Chart auditing
  • Complete registration process for lab testing
  • Scanning & faxing including faxing lab results to outside providers
  • Follows visit schedule including remaining in the home for the assigned time
  • Communicate with family related to schedule & schedule changes
  • Maintain appropriate levels of medical supplies according to established par levels
  • Manage inventory & management of supply of donations
  • Maintains storage area and office in an organized & efficient manner
  • Cleans & disinfect equipment as needed
  • Time/visit documentation is accurate, timely, thorough, & signed per departmental, state, & federal requirements
  • Participates and assists with quality improvement and LEAN efforts including data gathering & documentation
  • Willingness to accept new assignments
  • Flexible with schedules & duties
  • Works independently & able to multi-task
  • Duties as assigned by manager
  • Assists with running reports
  • Demonstrates competencies for all populations served

Required Skills/Abilities:

  • Strong verbal, written, & listening communication skills
  • Detail oriented
  • Must be able to operate a computer, phone, copier & medical equipment

Education and Experience:

  • High school diploma or equivalent
  • Licensed as a Certified Nurse Aide in the state of Indiana
  • Up to 3 months related experience required
