Nurse Practitioner/Physician Assistant - Primary Care

The Advanced Practice Clinician-Primary Care will be responsible for routine primary care visits. They will have access to advanced technology, including a robust health plan portal, EMR, and a proprietary decision support tool that synthesizes data from a variety of sources into actionable alerts that drive care.

• Providing primary care to patients
• Performs initial history and physical examinations and patient observations
• Orders, administers, and interprets appropriate diagnostic tests. Performs diagnostic and therapeutic procedures such as administering IV, wound care, etc. Administers vaccinations
• Communicates test results to patients and families
• Recording patient medical history
• Maintains a customer-driven passion for excellence and commitment to innovation and implementation of ideas that improve the healthcare of the patient population
• Develops care plans and ensures all patients have advanced care planning. Educates patients on various health related topics, such as disease management, medications, healthy lifestyle, end of life care, etc.
• Comply with all HIPAA regulations and maintain security of protected health information (PHI)

 Pay Base Range: $130,000 - $160,000 annually
