ABOUT THE ROLE:

The Clinical Care Coordinator for Assisted Living and Memory Care, who acts as our Health and Wellness Coordinator is a full-time position that manages the day-to-day health care operations for Assisted Living and Memory care units, ensuring that resident’s healthcare needs are met.  Guarantees that residents are treated with respect and dignity while maintaining quality care as their needs change.

Some of the duties include…

  • Ensures consistent quality resident care and service delivery throughout all shifts; this may require assuming the role and responsibilities of a Resident Care Associate to meet the residents’ needs and staff requirements.
  • Assists in the supervision of community-based licensed nursing staff, medication technicians, CMA’s and Resident Care Associates.
  • Provides training and education to Resident Care Associates on an on-going basis to include classroom in-services and situation-specific training.
  • Assists in the maintenance of resident charts and review of documentation performed by care-giving staff.
  • As allowed by the Nurse Practice Act, assists in assessing the health, functional and psychosocial status of residents.  Initiates individualized Personal Service plans as part of a multi-faceted treatment team and proactively manages care and services for each resident.  Evaluations the effectiveness and maximize the residents’ opportunity to remain in their environment.
  • Ensures and conducts Personal Service reviews and updates, according to FV and state requirements, to ensure residents’ service needs are met.
  • Assists in the achievement of occupancy goals by monitoring the health, safety and well-being of residents, proactively identifying those at risk for move-out.  Implements timely problem-solving.
  • Oversee and manage the continuity and consistency of medication training, pharmacy management and medication supervision and/or administration in the community.  Conducts periodic Care Associate medication audits per Freedom Village guidelines.
  • Shares on-call and manager on duty responsibilities/shifts as required.
  • Staff On-Boarding 

Qualifications 

  • Registered Nurse or Licensed Practical Nurse/ LVN
  • 3 plus years of relevant work experience with supervisor experience preferred.
  • Basic computer proficiency.
  • Can communicate effectively with residents, families, staff, and visitors.
  • Maintain the highest level of confidentiality.
  • Can pass a pre-employment drug screen and criminal background check.
