About the role

Description

The Independent Living Specialist should have a passion for engaging teens and the young adult population to assist with their transitioning into successful independent adults. The Independent Living Specialist provides case management for all post 18+ clients participating in Extended Foster Care, Postsecondary Education Support Services and Aftercare programs. The Specialist will monitor and ensure client eligibility for program services, provide monthly home visits and contacts with providers as well as clients. The Specialist will assist the young adult population with appropriate community services and information to include but not limited to Social Security, Agency for Persons with Disabilities, Education, Employment and Health care providers. The Specialist is required to have certification as a child welfare case manager by the Florida Certification Board to comply with DCF guidance. The Specialist will be responsible for providing court documentation, attending all court hearings, and staffing pertaining to the young adult. This candidate should have strong organizational skills, critical thinking, and the ability to work in a trauma-informed manner.

  

KEY RESPONSIBILITIES:

  • Is the primary point of contact for all post 18 Independent Living Clients in the 18+ Independent Living Programs. 
  • Follow up on all young adult requests for services or inquiries regarding eligibility for services.
  • Ensure and monitor client eligibility for services; provide and assist clients in the completion of appropriate applications for services.
  • Assist clients in identifying and locating appropriate supportive documentation to secure entry into the18+ Independent Living Programs.

Requirements

EDUCATION / EXPERIENCE:

Child Welfare Certification and Bachelor’s degree in a social service-related field preferred.
