Registered Nurse - Field RN (Float) - 5 Boroughs

FlexStaff is seeking an experienced Registered Nurse for a Field RN Float position on our client's team, a leader in home health care. This is position will be covering cases within the 5 boroughs. 

The Field RN Float is a Registered Professional Nurse who will manage all aspects of patient care related to services provided in the home. 

Responsibilities include: conducting home care assessments, home visits, completing all required documentation, communication with PCP, updating medication profiles, coordination of care with other providers, review and completion of case communication notes, discharge planning activities and other duties as assigned. 

Requirements: 

• NYS RN License
• Home Health Care experience preferred 
• Must have hospital experience
• Candidate must drive
