Full-Time: Monday-Friday 8am-5pm
SUMMARY
The RN Admission and Health Monitoring role involves providing skilled nursing services to patients with mental health and substance use disorders. The RN will conduct assessments and recommend a patient’s care pathway within the agency. RN will conduct initial health assessments, social determinate of health assessments, mental health and substance use assessments and complete internal and external referrals. RN will at times develop nursing care plans, monitor patient progress, and collaborate with other healthcare providers to ensure comprehensive care.
ESSENTIAL DUTIES AND RESPONSIBILITIES
- Patient Assessment: Conduct comprehensive assessments of patients' psychiatric history, physical health, substance use, and social functioning.
- Patient Access: Coordinate patient pathway through organization based on comprehensive assessment and initial evaluation. Set patient up for success with access to services within 24-28 hours.
- Medication Management: Support patient needs and communicate with prescribers as necessary.
- Treatment Planning – Develop nursing plan for patient based on needs identify during initial assessments.
- Crisis Intervention: Provide on-call crisis intervention and support during emergencies.
- Patient Education: Educate patients and their families about mental health and substance use disorders, treatment options, and self-care strategies. Provide any and all appropriate health related education such as HIV, TB, Hepatitis, smoking cessation.
- Coordination of Care: Collaborate with internal and external partners including other community agencies, primary care providers, and other healthcare professionals to coordinate patient care.
- Documentation: Maintain accurate and timely documentation of initial assessments, patient care and progress in the electronic medical record.
- Community Outreach: Develop relationships with community providers to promote access to behavioral and physical health resources.
- Health Monitoring: Support patients by providing health monitoring sessions to review vitals, ekgs, education, TB, HIV, Hep C, tobacco cessation and other health related care and education as appropriate and necessary.
- Work collaboratively with staff and other healthcare providers to achieve optimal patient outcomes.
- Interact with individuals from diverse backgrounds in a culturally competent manner.
- Attend and actively participate in staff and departmental meetings, treatment team meetings and supervisory meetings.
- Work at various agency clinics as needed.
- Proficiency in electronic medical records and other relevant software.
- Maintain required licensure and certification.
- Demonstrate commitment to the agency's mission and goals.
- Performs other duties as assigned or that may develop.

EDUCATION and/or EXPERIENCE
Required: NYS RN License in good standing
Preferred: bachelor’s degree in nursing. Minimum of 2 years of experience in a healthcare setting, preferably in mental health or substance use services.
CERTIFICATES, LICENSES, REGISTRATIONS
Required: Must maintain valid NYS RN License, driver’s license and acceptable driving record
