Home Health Scheduler General Summary: Responsible for coordinating patient shifts and visits, maintaining and upkeep of scheduling records and logbooks and patients or family members. This includes the accurate and timely communication of scheduling changes between office and field staff and aide roster. Patient Population: N/A Essential Functions:
- Promote /exemplify the Company's mission, vision, and values.
- Ensure timely staffing and scheduling visits for field staff in their appropriate areas per physician orders, insurance authorization, and/or patient/client requests.
- Ensure timely follow-through with staff, patients/families, and physicians, which may include but not be limited to the Start of Care (SOC) report, Schedule Calendar Reports, emails, faxes, labs, etc.
- Maintain a current client roster with necessary information.
- Ensure caregivers have met all HR requirements and possess the skills for their assigned patients.
- Communicate effectively with clients, their families, team members, and other healthcare professionals.
- Promote the Company's financial success by maintaining proper caregiver pay rates and limiting overtime hours.
- Demonstrate commitment and professional growth by participating in in-service programs and maintaining/improving competency.

Team Assistant
Position Summary: Provides clerical support to the Clinical Manager in managing the day-to-day team operations through assigned workflow processes driven primarily by cross-matrix EMR reports. Supports the home health team by anticipating and responding to team processes related to scheduling, and complete, accurate, and timely flow of patient information. This includes, but is not limited to, patient visit scheduling; MD orders management; assigned workflow processes; and general clerical support to Clinical Manager and assigned clinical Team functions. Responsible for keeping the flow of team processes organized to expedite communication and support policies and procedures that ensure smooth, continuous team operations.
ESSENTIAL FUNCTIONS: Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.
• Monitors assigned reports and follows up on unresolved discrepancies.
• Distributes and/or provides communications to appropriate staff as directed.
• Maintains assigned tracking systems with accurate/up-to-date information.
• Receives, addresses, triages, and relays messages for assigned Team field staff (incoming faxes including physician orders, lab results, etc.).
• Participates in quality management activities as assigned.
• Facilitates maintaining, updating, and communicating visit schedules for Team field staff.
• Assists with pre-bill audit activities as assigned. • Assists with weekend patient scheduling and on-call coverage.
• Supports Agency’s customer service reputation by promoting and maintaining a responsive, knowledgeable, and accessible approach to all customer
contacts.
• Other duties as assigned by Clinical Manager.

Requirements
MINIMUM QUALIFICATIONS
- High school diploma and experience in computer processing preferred.
- Active LPN State License required.
- Above average word processing/data ent
