Join our dynamic healthcare team as an Associate Director of Case Management, where leadership meets compassionate patient care in a truly collaborative environment. We're seeking an experienced professional with a Master's in Social Work (with licensure) or RN License who brings case management expertise and a passion for leading others to excellence. You'll manage front-line case management staff and discharge planners, guide your team through complex discharges, optimize patient throughput and length of stay, and handle recruitment, scheduling, and staff development—all while utilizing familiar tools like Microsoft Outlook, Teams, Excel, and Cerner EHR. POSITION SUMMARY
This position provides managerial and clinical support to patients and staff. Uses depth and breadth of clinical management knowledge to ensure the provision of high quality, safe, and cost-effective patient care. Demonstrates care coordination expertise and oversees the leadership of clinical, financial, and personnel management of the Care Coordination department. This position supervises employees and participates in selection, orientation, counseling, evaluation and staff scheduling. Maintains clinical, leadership and post-acute care services knowledge and competency to evaluate outcomes related to delivery of Care Coordination services.
CORE FUNCTIONS
1. Oversees Care Coordination personnel as assigned. Provides and/or facilitates Care Coordination and/or other related activities and serves as a resource to other staff members. Promotes utilization management through interdisciplinary patient care and activation of the discharge plan per the care delivery model.
2. Directs personnel actions including recruiting, new hire actions, interviewing and selection of new staff, salary determinations, training, and personnel evaluations. This position also participates in the development of Care Coordination goals and objectives in accordance with company standards. Ensures all goals and objectives are met timely and effectively. Hires, trains, conducts performance evaluations, and directs the workflow for the staff. This position is also accountable for participating in the development and implementation of department goals and objectives
3. Serves as a resource and provides leadership assistance to achieve optimal clinical, operational, financial, and satisfaction outcomes.
