Program Description:

@HOME delivers high-quality primary care to Medicare patients in their home. The @HOME program takes a team-based approach to providing outstanding patient care to the population attributed to Oak Street Health’s Managed Service Organization (MSO). Our Care Teams - consisting of a Provider, Medical Assistant/Scribe (CIS/MA), Behavioral Health Specialist (BHS) and Mobile Operations Coordinator (MOC) - build individual relationships across a panel of about 75 patients, seen monthly. The Care Team is responsible for delivering excellent, high-touch primary care, and coordinating the care of our patients throughout the healthcare delivery system.

Role Description:

Population Health drives both our clinical and economic model and is focused on delivering high-quality, cost-effective care.

The Population Health Director will be instrumental in designing and implementing key Population Health priorities. They will work on programs and interventions, targeted to the right patients, to improve quality and cost of care. They will be an effective cross team leader, working side by side with providers, care teams, operations colleagues, and corporate colleagues.

Content areas for the Director will include, but not be limited to, accurate risk adjustment, transitions-in-care, pharmacy and medication adherence, and HEDIS / Stars. This will require building relationships with our CIS/MA cohort/leadership as well as clinicians, including nurse practitioners, pharmacists, licensed clinical social workers, and others.

This is a fast-paced, high visibility role with a growing program to ensure that we are driving innovation and improvement rapidly and consistently within a division. This role will be aligned with a geographic division to rapidly drive results.

This role will report directly to the Regional Vice President.

Core Responsibilities:

  • Own the execution and the ongoing refinement of Population Health programs in the local division
  • Work closely with functional leaders in the central office to assist with strategy and program design
  • Work side by side with regional leaders (Senior CIS/MA, Lead CIS/MAs) and Care Teams to ensure effective implementation of key clinical and business priorities
  • Accountability to key population health metrics, including quality measures, utilization measures, and financial measures
  • Other duties, as assigned

What are we looking for? We're looking for a motivated, experienced leader with:

  • Bachelor's degree required
  • Graduate degree preferred but not required (MBA, MPH)
  • 7-10 years of prior relevant experience. 5+ years specific experience in healthcare / population health / working with healthcare providers; or other similar fields
  • Ability to build the trust and confidence of a wide range of stakeholders
  • Strong leadership and influencing skills
  • Incredible level of organization and detail orientation
  • Analytical and data savvy. Ability to develop business cases, ROI analysis, and financial analysis
  • Someone who connects the dots between teams - a proactive communicator who puts individual projects into broader organizational objectives
  • Expert in Excel, Google Apps, project management
  • Pleasantly persistent approach
  • Quick learner comfortable with a high level of ambiguity
